Office Policies

Billing

  • Payment is due the same day as services received.

  • Our office does not do back-billing. Insurance must be presented the same day as services rendered to be considered valid.

Cancellation Policies

Follow-up Visits

  • We offer free prescription rechecks for glasses and contact lenses within 90 days of the original exam date.

  • Contact lens fittings must be completed within 90 days of the original exam date.

    • After 90 days, another exam will be required.​

Late Policy

  • Please arrive 10 minutes prior to your scheduled exam time to complete pre-testing and any necessary paperwork.

    • Paperwork will also be sent to you prior to your appointment.​

  • If you arrive more than 10 minutes after your scheduled exam time, you may have to reschedule.

Cancellation Policy

  • If you need to cancel your appointment, please do so at least 24 hours prior to your appointment. 

    • Cancelling your appointment less than 24 hours prior to your appointment more than 2 times within one year may result in a $40 rescheduling fee.​

    • We reserve the right to dismiss any patient from our care who violates this policy more than 3 times within one year.

  • Our office will attempt to contact to you via phone call and text message prior to your appointment to confirm and verify any insurance information.  If you do not confirm your appointment within 24 hours of your scheduled appointment, your appointment may be cancelled.
